Important Security Notice - 11/1/2017

Advocate Health Care is in the process of standardizing logins across the enterprise. As a result all non-conforming user accounts that contained a "." period in the username were disabled on Tuesday October 31st, 2017.

What do I need to do to get my account re-enabled?

For users that have an office or practice manager.
Contact your office or practice manager and make them aware that you have a non-conforming username that has been disabled. Your practice manager should download a "New Office Staff Request" form from here and submit by fax to 309-268-2139. For questions call 309-268-5510 to speak with an Advocate BroMenn HIT Analyst. If prompted, please leave a voicemail message and an HIT Analyst will call you back at their first opportunity.

 

For users that do not have an office or practice manager.
For users and offices familiar with the new office staff request process, download a "New Office Staff Request" form from here and submit by fax to 309-268-2139. For those unfamiliar with the process or if you have questions call 309-268-5510 to speak with an Advocate BroMenn HIT Analyst. If prompted, please leave a voicemail message and an HIT Analyst will call you back at their first opportunity.
